Special Conditions:
Registered as a Social Worker with Social Work England.
Ability and willingness to travel between locations.
Availability for occasional work outside normal office hours.
Compliance with the Government’s Code of Practice on the English Language Fluency Duty for public sector workers.
Main Purpose of the Job Role:
Improve outcomes for adults with care and support needs and carers.
Undertake effective strengths-based assessments, planning, and direct work in alignment with the Care Act 2014 and statutory guidance.
Conduct safeguarding duties to protect adults with care and support needs from abuse or neglect.
Practice social work ethically and transparently in accordance with professional standards.
Role Specific Duties and Accountabilities:
Practice within legislative frameworks and policies.
Conduct strengths-based assessments promoting self-determination and independence.
Co-produce care and support plans and review their effectiveness.
Manage workload effectively, addressing various challenges.
Advocate for adults with care and support needs, ensuring their voices are heard.
Collaborate with colleagues and maintain accurate records.
Lead statutory safeguarding enquiries, planning, and reviews.
Support team members’ professional development and progression.
Practice in an anti-discriminatory and anti-oppressive manner, promoting diversity and equality.
